WAIVER & RELEASE and ENROLLMENT TERMS
Please review the Waiver/Release below. Submitting your registration indicates agreement to Warrior Fusion Waiver/Release and Enrollment Terms.
As the parent or legal guardian, I do grant permission for the student named in this registration form (hereby referred to as "my child") to participate in the activities of the Young Warriors Program, conducted by employees of Warrior Fusion. I am fully aware of and appreciate that the course in which my child will participate involves an inherent risk of accidental harm, injury, or even death. I understand this assumed risk may arise from, but is not limited to, the instruction, explanation, practice, and training in the subject matter of the course. I therefore agree to assume any risk and hold harmless Warrior Fusion and its owners, officials, instructors, employees, other participants, sponsors, and agents. I do hereby for myself, and all others who may have a similar claim, waive, release, and forever discharge any and all rights, claims, and causes of action whatsoever which may arise now or in the future against Warrior Fusion, its owners, operators, agents, representatives, or host school/institution for any and all damages my child may sustain while participating in this course.
I certify that my child is in good health and physical condition and has medical insurance that covers his/her participation in this course. I hereby give my consent to Warrior Fusion to provide, through a medical provider of its choice, customary medical attention, transportation, and emergency medical services as warranted during the course of my child's participation.
I give Warrior Fusion and its agents permission, without compensation, to take and publish photographs of my child for promotional purposes.
I understand that uniform fees will not be refunded and that the uniform is for my child to keep. I also understand that course fees are not transferable and will not be refunded, with exception given if my child develops a catastrophic, long-term medical condition that is documented by a medical doctor and prevents my child from continued participation in the program. In that instance, a pro-rated amount can be refunded.
FALL 2017 KARATE AT ALL SITES EXCEPT TWO RIVERS is now FULL!
Register for karate at Two Rivers PCS 4th St. or Young campuses through Springboard Afterschool. The session begins on September 12th.
Karate at Ludlow-Taylor is FULL. The session begins on September 6th.
Karate at SWS is FULL. The session begins September 11th.
Youth karate at the Hill Center is now FULL. Drop-in for $20 per class only with advance instructor permission.
Fall karate at the Hill Center is open to new and returning students of all belt levels, ages 5 and up. Classes take place on the following days and times September 6th through December 13th (with no class on October 7, 28; November 22, 25):
FULL - BEGINNING KARATE (age 5 and up, yellow belt and below) - Saturdays @ 10:00 AM
FULL - MIXED BELT KARATE (new and returning students, all belt levels) - Wednesdays @ 5:00 PM and Saturdays @ 11:00 AM
FULL - ADVANCED KARATE (upper belt students, or by instructor permission) - Saturdays @ noon; requires concurrent enrollment in regular class at any site
Uniforms (Gi - pants, jacket, and belt) are required and can be purchased through our warehouse. Advanced students should bring sparring gear to the Advanced class.
